Customizable Chat Request Forms

Before a chat begins, visitors are presented with a pre-chat form to fill out with basic information about themselves or the chat request.

By default, Name, Email and Question are hard coded into the form and cannot be changed; however, the names of the fields can be edited.
The information inputted by the customer to the Request form is displayed in the chat request before a chat begins and is manageable per department.
Forms may also be skipped to go straight to the chat window.
