Request Form Customization
Below are examples of the 4 options that are available for Request form customization.

You can add as much or as little to your form, its your choice. By default Name and Question are hard coded and cannot be changed.
To add your options, login to your control panel via the following link: Login
Next, from the menu, select Preferences >> Request Form Customization
Next, click on the "Add New" button to begin.
You will be presented with the Add new item menu, Item Caption is the name that will be displayed next to the form item. Item type, see above for descriptions. Number is how the form item will be ordered, so if you want the item to be on the top, you would give it a number of 1.
Item Type, Drop down list has an extra step involved when adding. After you add a drop down list type, you will notice now that under the values column, you have a link. This is where you will add your drop down list selections. Click on the Values link to add your drop down selections.
Click on the "Add New" button to start.
Next, you can add the Value (this is what will be displayed in the drop down list.) The Number is used to order the drop down items, 1 being the top most item.
You can add as many drop down items as you wish.
The information inputted by the customer to the Request form is displayed to you in your Operator application when you accept the chat request.
